PROVIDER SERVICES - Login Information Request
|
There are two ways to request your own unique login information (user name and password). You will need this information any time you want to update your provider information for Aging Projects, Inc. We suggest that you keep these in a safe place that’s accessible when needed.

- Get an automated response within a few minutes (usually) by filling out a form.
- Email the webmaster. You will get a response within 24 hours—usually sooner.
Note that once you have your user name and password, you can UPDATE your provider information to change your user name and/or password to something more easily remembered.
