Provider’s Statement of Service(s):
The Y is dedicated to improving the overall health of the community. Our Population Health team takes several approaches to improving health outcomes for people with chronic conditions or diseases. For adults over 65 or older with two or more diagnosed chronic conditions, Medicare Part B may cover the fees of our evidence-based programs. Our team of lifestyle coaches and health navigators will work with the patient and their primary care provider to better manage chronic conditions and improve health. We also recognize 1 in 2 older adults suffer from malnutrition which increases the risk of chronic disease. Our Healthy Living Mobile Kitchens and Mobile Market distribute fresh food along with information on how to store it and practical, cost effective cooking sessions. We now reach more than 300 families each month.
We work to reduce social isolation with plaques over 60% of our older adult population by providing educational workshops and events to bring older adults together in a social setting to enjoy like minded activities which leads to improved mental health. Our Woodfin branch offers a monthly caregiver support group for caregivers caring for their loved ones suffering from chronic physical and mental diseases and our Y staff has been trained by Dementia Friendly WNC so they are equipped to empower older adults suffering from neurocognitive disorders live their fullest lives through enjoying the many services the Y offers.
